New Juror Questionnaire

The Macomb County Clerk/Register of Deeds has implemented a new computer system for jury service. The new questionnaire lets you save the postage cost by replying online if you want.
You may also request to excuse or postpone yourself when necessary on the Web under certain court-approved circumstances. You may still be required to submit documentation verifying the reason for your request.
Clerk staff now process questionnaires more quickly with less staff time by digitally scanning them. Please use a blue or black pen only.
RESPONDING ONLINE
In order to complete the questionnaire online, you must use Internet Explorer as the Web browser. (It will be compatible with other browsers in the future.)
You will get a faster response to your request for postponement or excusal, if you make your request online. However, you may also U.S. Mail a written request with your questionnaire. If replying by U.S. Mail, please tear along perforation and keep top portion of your questionnaire. Bring the top portion with you on your service date for those summoned to the 16th Judicial Circuit Court.
MEDICALLY UNABLE
If you are medically unable to serve as a juror, please complete and return the questionnaire and include a letter from your physician indicating the reason why.
HIGH SCHOOL OR COLLEGE STUDENT
If you are a full-time student, you still must complete the questionnaire. Return it with a copy of your current class schedule.
If the student is unable to complete the questionnaire (attending school, living outside of Macomb County), a parent must indicate that their child is a student with the name of the college or high school, sign their name indicating that they are the parent, date it and return it in the enclosed envelope or online. You will receive a response.
OVER THE AGE OF 70
If you are over the age of 70, you are not required to serve on jury duty, answer yes to question #5 online or on the paper questionnaire and return it. You will receive a response.
NO DAYCARE
Complete the questionnaire and include a detailed letter as to why you are unable to make arrangements for daycare. You will receive a response.
EMPLOYER REQUEST
Complete questionnaire and return with a letter from your employer explaining in detail why you are unable to serve. Your employer can not prevent you from coming to jury duty per Michigan Statute. You will receive a response.
TRAVEL
Complete the questionnaire and return it with documentation showing that you will be out of town on your date of service. Documentation may include: reservations, airline tickets, confirmation, or e-tickets. If driving please include an itinerary with as much detail as possible, where, when, and also an alternate date you are able to serve.
ADDRESS CHANGE
If your address has changed within the county, please indicate on the questionnaire and include the new address. If you have moved out of the county, please indicate the new address and include a copy of your changed driver license. You will receive a response if you have moved out of the county.
